Dependent Scholarship Application
Purpose
Use the Dependent Scholarship Application to apply for a scholarship for a child or other dependent. After the first time, you do not need to apply again unless your dependent changes schools. If you have more than one dependent eligible for scholarships, complete a separate application for each one. If you wish to apply for a scholarship for yourself, use the Associate Scholarship Application.
Your dependent is welcome to access Oak Tree to help you complete this application and other forms.

Button Functions
Three buttons support the form:
Cancel - Click this button to return to the Associate Dashboard. When clicked, the form displays a warning dialog box. If you click OK, the system takes you to the Associate Dashboard.
Save as Draft - Click this button to save the form in its current state. The form is in draft status. You can continue editing the form by clicking the Create Associate Scholarship button again. Until you submit the form, once you start creating the form, you can edit only this version of the form.
Submit - Click this button to submit the form for review by the Acorn Scholarship Fund. The system then checks that all the fields have valid fields. Required fields must be completed. Fields with particular formats like dates and money must have correct formats. If the form is completed correctly, the status of the form is changed from draft to submitted. The system sends an alert to the Chair of the Scholarship Committee, so that the Chair can verify eligibility and present the application to the Fund's board of directors for approval. Once the form is submitted, you can no longer change it. If you click on the Create Associate Scholarship button, you get a new version of the form.
You can view the form by locating it in the table of forms labeled My Forms. Click the View Form link to bring up the form. You can then view the form, but you will not be able to change it.
If your circumstances change, for example you are unable to attend the school, you can withdraw the form by clicking the Withdraw button. After this, your application is no longer considered for a scholarship. At this point, you can submit a new application with any updates.
What Happens After You Submit an Application
When you submit a Dependent Scholarship Application, the Chair of the Scholarship Committee reviews the application for correctness. If it needs updating, the Chair flags it for update. The application's status is changed to draft, so you can edit it if you wish to fix the issues identified by the Chair. If the application is satisfactory, the Chair checks with Cypress Human Resources to confirm you meet the eligibility requirements. Then the Chair submits your application to the Fund's board for approval. If the board approves the application, you will be notified by an alert.
At this point, the Chair will assign a mentor to you. Your mentor will contact you about the next steps. In particular, your mentor will ask you to sign a scholarship agreement and will give you an award letter.
As soon as you have an invoice from your dependent's school, you need to submit a Dependent Payment Form to instruct the Fund's Treasurer to send the scholarship check to your dependent's school.
